Position Highlights

Position Summary

The clinician is responsible for direct patient care services to medically complex patient populations. They also serve to educate and collaborate with medical and specialty providers on the management of mental health disorders.

Essential Duties & Responsibilities

Under limited supervision.

  • Provides direct clinical care to patients with medical specialty co-morbidities.

  • Participates in direct care to assess needs and coordinate services to appropriate population (children, adolescents, adults and families)

  • Provides initial mental health assessments resulting in measurement based care planning

  • Assesses using standardized screening tools, measures, etc

  • Provides brief, solution focused therapy

  • Provide group psychoeducation/treatment related to specific diagnoses/presenting issues

  • Provide information and referral services

  • Participate in care coordination

  • Maintains daily/weekly scheduled appointments in electronic records systems

  • Provide therapeutic services to patients and their families

  • Work in collaboration with multidisciplinary team members including external partners and agencies

  • Completes required documentation and paperwork including biopsychosocial assessments and treatment planning and implementation in a timely manner

  • Participate in our high risk program

  • Demonstrates cultural competency and commitment to patients including LGBTQ+ community

  • Maintain case load dependent on need

  • Provide supervision and clinical oversight to master’s level students/interns

  • Provides mentoring and training to newly hired staff as appropriate

Minimum Qualifications

  • Licensed independent practitioner, licensed in the State of CT

  • Excellent communication and people skills

  • Extremely organized and detail oriented

  • Proficiency in computer and information systems


Preferred Qualifications

Experience working in medical field

Experience working with care management reimbursement codes

Experience with electronic health records, specifically Epic
