JOB SUMMARY:Under the general supervision of the preceptor, the incumbent will manage time effectively to fulfill practice responsibilities, present oneself as an assertive member of the profession, and others as specified in the goals and objectives outlined for the residency program by the American Society of Health-System Pharmacists.
Provide pharmaceutical care to patients with acute, subacute, or ambulatory needs include interdisciplinary rounds, patient education, and therapeutic drug monitoring. Perform or assist to perform adverse drug reactions evaluation, drug monograph development, medication error evaluation, publication of newsletters when appropriate, drug utilization review, pathway development and participation in investigational review board. Cross trains in all areas of pharmacy practice. Conducts inservices and provides drug information to patients, physicians, nurses, members of the multi-disciplinary team, etc.
