Compensation: $56–$60/hour (commensurate with experience)

Position Summary
The Admissions Registered Nurse (RN) – Hospice is a key part of the patient onboarding experience, responsible for evaluating and admitting patients into hospice care. This role involves conducting clinical assessments, educating families, developing initial care plans, and coordinating interdisciplinary services. You will serve as a trusted clinical resource while guiding patients and families through sensitive transitions with dignity and empathy.
Key Responsibilities
Conduct comprehensive assessments for newly referred hospice patients
Develop and revise individualized care plans based on patient conditions and goals
Initiate and document the plan of care, medications, and nursing diagnoses
Collaborate with physicians and interdisciplinary team members
Teach families and caregivers self-care techniques and hospice principles
Provide support for activities of daily living and comfort measures
Participate in case conferences and interdisciplinary group meetings
Serve as a point of contact for patient admissions and transition coordination
Required Qualifications
Graduate of an accredited school of nursing
Active RN license in California
Current CPR certification
1–2 years of recent acute care experience
1–2 years of recent home health or hospice experience
Physical ability to lift and transfer patients and conduct prolonged walking/standing
Visual and auditory capacity to perform nursing duties
Licensed driver with a reliable, insured vehicle
Preferred Qualifications
Bachelor’s degree in Nursing (BSN)
At least 1 year of hospice experience
Training or experience with holistic, complementary, or alternative modalities
