Position Summary
The Admissions Registered Nurse (RN) is a critical member of the hospice care team, responsible for assessing, admitting, and initiating care plans for patients entering hospice services. This role offers a unique opportunity to provide meaningful, direct support to patients and families navigating end-of-life care, while collaborating with a dedicated interdisciplinary team.
Key Responsibilities
Conduct initial and comprehensive patient assessments in-home or at facilities
Initiate individualized plans of care in coordination with physicians and hospice team
Develop nursing diagnoses and deliver patient-centered palliative interventions
Educate patients and families on hospice philosophy, care practices, and self-care strategies
Supervise and delegate care to hospice aides, ensuring adherence to care plans
Communicate changes in patient condition and obtain new orders as needed
Serve as a liaison between patients, caregivers, and the hospice team to ensure seamless admissions and continuity of care
Required Qualifications
Graduate of an accredited school of nursing
Active Registered Nurse (RN) license in California
Current CPR certification
1–2 years of recent acute care experience
1–2 years of recent home care experience
Physical ability to perform prolonged walking, standing, and lifting/transferring patients
Strong verbal/written communication and clinical documentation skills
Valid driver’s license and operational, insured vehicle
Preferred Qualifications
Bachelor’s degree in Nursing (BSN)
1+ year of hospice care experience
Familiarity with holistic, complementary, or alternative care approaches

Proficient in medication administration and nursing procedures
Able to communicate with empathy and clarity in sensitive situations
Effective in delegating and supervising direct care staff
