Job Summary:

The Licensed Practical Nurse (LPN) will provide direct patient care as part of a larger medical team, working under the supervision of a Registered Nurse. Key responsibilities include monitoring vital signs, assisting with hygiene, documenting medical histories, and delivering basic medical treatments. The LPN will be expected to collaborate with other healthcare professionals to ensure comprehensive patient care.

Key Responsibilities:

  • Implement the care plan developed by the Registered Nurse.

  • Provide accurate and timely documentation of patient care.

  • Communicate patient needs to the Registered Nurse and other healthcare providers.

  • Educate patients and families about the diagnosis, treatment, and care plan.

Required Qualifications:

  • Graduation from an accredited LPN program.

  • Minimum of one (1) year of recent experience in acute care or home health.

  • Current state LPN licensure.

  • Valid CPR certification (BLS).

  • Valid driver's license and insured vehicle.

  • Strong observation, communication, and problem-solving skills.

Preferred Qualifications:

  • Associate’s degree in Nursing.

  • One (1) year of home health care experience.

  • Valid CPR certification.

  • Valid driver’s license and auto insurance.

  • Ability to work independently and manage a flexible schedule.

Travel and Work Schedule:

  • This position requires daily travel within the coverage area.

  • Flexible 5-day workweek with weekends off. On-call rotation required (one weekend per month and one day per week).

  • Typically no night shifts.
