A Brief OverviewFacilitates all day-to-day project operations including taking incoming calls, coordinating schedules between various specialties, making referrals, managing workflow operations in the clinic, and gather relevant information.
What you will do
- Assists clients in eliminating barrier to timely, quality care through all phases of healthcare including: prevention, detection, diagnosis, and treatment.
- Completes needs assessment of all clients and documents within electronic medical record as appropriate.
- Schedules and makes referrals to appropriate resources and services, documenting within electronic medical record as appropriate.
- Provides ongoing support and follow-up per outlined protocols and documents within electronic medical record as appropriate.
- Provides patient education and educational resources.
- Provides clinical management of clients calling in with symptoms and medical concerns, including appropriate triage with documentation in electronic medical record as appropriate.
- Assists leadership in planning, development, implementation and evaluation of goals and objectives.
- Assists leadership with data collection, analysis, and reporting.
- Facilitates access to care including connecting patients to appropriate appointments at first contact with documentation in electronic medical record as appropriate.

Required Education, License/Certification, or Work Experience:
- Registered Nurse (RN) - Board of Nursing An active license in the state of practice Upon Hire
