As a Registered Nurse Case Manager, you will directly impact patient outcomes by delivering personalized, at-home care that promotes independence and well-being.
Pay Range
$55 – $65 per hour, commensurate with experience.
Schedule Options
Monday to Friday, 8:00 AM – 5:00 PM, with rotating weekend coverage every 4th weekend
Monday to Friday, 11:00 AM – 7:00 PM
Coverage Area
Specific territories within the greater San Diego area; flexibility required.
Key Responsibilities
Conduct comprehensive initial assessments of patients and families to determine care needs.
Develop, implement, and revise individualized care plans based on nursing diagnoses and patient conditions.
Deliver preventive and rehabilitative nursing care, including medication and treatment administration.
Provide education and counseling to patients and families regarding care plans and wellness strategies.
Coordinate discharge planning and ensure seamless transitions across care settings.
Document and report changes in patient conditions and treatment responses.
Collaborate with interdisciplinary teams to ensure optimal patient outcomes.
Qualifications
Current and valid RN license in the state of California.
Minimum 1 year of recent acute care experience in an institutional setting.
BLS certification with hands-on CPR required.
Valid driver’s license and auto insurance.
Preferred: Bachelor’s degree in Nursing and 1 year of home health experience.
