Responsibilities:
The Care Transitions Nurse develops strong relationships with Hospital/Health System providers, Hospitalists, Physicians, Home Health and others as identified within assigned geographic area to grow patient referral volume through the delivery of market-leading patient outcomes.
*Expand customer base by increasing nursing center occupancy and skilled mix from assigned hospital accounts and/or mining unassigned hospital accounts.
*Make patient acceptance and placement decisions on behalf of Genesis and communicate decisions to referral sources based on published performance standards.
*Ensure safe and effective transfers of patients across the care continuum serving as a bridge between the hospital and nursing centers, staff and the patient and/or family with the objective to reduce re-hospitalizations and improve customer satisfaction.
Qualifications:
*RN license in state of employment is required. Must be willing to obtain licensure in additional states as requested.
*Must have a valid driver's license.
*Five to seven years of recent clinical experience is required.
*Must be an expert working in automated, highly competitive environments navigating multiple applications to source patients, communicate acceptance and analyze case while synchronizing data in internal systems to communicate internally regarding patient status.
*Must be able to work flexible hours to meet hospital needs.
*Case management experience is required.

Restrictions apply based on collective bargaining agreements, applicable state law and factors such as pay classification, job grade, location, and length of service.
Posted Salary Range: USD $100,000.00 - USD $110,000.00 /Yr.
