Job Summary

Collaborating through the multidisciplinary patient care team, the Behavioral Health Therapist ensures coordination of patient care through the development of patient population specific plans of care to address the physical and psychosocial needs throughout the treatment and post-discharge planning process. The Therapist leads educational group, individual and family therapies focused on essential skills patients need to manage their illness as well as their social environment. The Therapist documents patient participation, progress and changes in condition, contributes to the formulation of individualized plans of treatment, and performs regular assessment of patient progress to ensure maximum therapeutic benefit. Assists in the collaboration, research, development, implementation, revision and reporting of the therapy program. Provides assessment and crisis intervention; plans, organizes and/or initiates social work services for patients and families. Provides long-term care planning , patient education and arranges for post-hospitalization services in accordance with the provision of care after discharge.

Essential Functions
  • Timely completion of functional assessments on assigned patients; demonstrates responsible decision making in planning, providing, and delegating of patient care based on assessment; participates in multi-disciplinary treatment team meetings (Master Plan and Reviews).
  • Timely communication with patients, family members, medical care providers, community agencies and referral sources; communication and follow through on issues or concerns and communicates information using appropriate chain of command.
  • Actively participates in patient identity and treatment plan confirmation according to established policy and procedure.
  • Demonstrates knowledge and care standards for all patient populations; exhibits therapeutic communication skills during patient care; assumes responsibility for patient care, safety and comfort while in the therapists' care.
  • Ensures accurate, timely communication among caregivers (e.g. handoff/report, SBAR, timely communication to charge nurse/supervisor of issues) to include appropriate chain of command when indicated; establishes and maintains two-way communication with peers, staff, physicians and administration.
  • Documents accurately and timely according to facility and regulatory standards; utilizes computer-based programs to aid in development of plan of care and interventions and in daily administrative scheduling.
  • Orients, instructs, and trains assigned personnel and/or students.
  • Collaborates in the order of treatments, procedures, testing, scheduling and transportation of patients to ensure timely services throughout all ordered procedural, patient care, and diagnostic testing department; assumes responsibility for appropriate delegation of patient care tasks and oversees care provided by delegates.
  • Enhances professional growth and development through participation in educational programs, current literature, in-services, and meetings.
  • Timely reporting and documenting of unexpected incidents or adverse events, including participation in investigations and completion of incident reports as applicable.
  • Participates in the department's quality assurance and improvement plan; responsible for compliance with department, regulatory, and facility policies, procedures and regulations.
  • Performs other duties as assigned.
  • Complies with all policies and standards.
Qualifications
  • Master's Degree In social work, psychology or counseling required
  • 1-3 years Prior Behavioral Health Experience preferred
Knowledge, Skills and Abilities
  • Must have a strong working knowledge of managed care and community resources and resource management relating to complex medical concerns.
  • Knowledge and expertise in working with patients with mental health concerns and psychosocial stressors as relates to facility service lines.
  • Knowledge of human behavior, symptoms of mental illness and appropriate therapeutic interventions, de-escalation and management strategies, group dynamics and stress management techniques are required.
Licenses and Certifications
  • ACLS - Advanced Cardiac Life Support preferred
  • LMSW - Licensed Medical Social Worker - State Licensure state specific preferred or
  • LPC-Licensed Professional Counselor preferred
  • BCLS - Basic Life Support required
