Position Summary/Department Summary: We are seeking a compassionate and skilled Clinical Social Worker to join our multidisciplinary team in the Home Parenteral Nutrition Program within the Gastroenterology Department. This is a 20-hour position which involves providing a range of clinical social work services and case management to patients with complex medical needs.This role requires specialized support and comfort in areas including parent coaching, child protection concerns, school advocacy, chronic illness adjustment, and family dynamics.
