The role reflects appropriate knowledge of RN scope of practice, current state requirements, CMS Conditions of Participation, EMTALA, The Patient Bill of Rights, AB1203 and other Federal or State regulatory agency requirements specific to Utilization Review and Discharge Planning. The Care Manager partners with the medical staff, utilizes scientific evidence for best practices, and relevant data to manage the care of the patient over the continuum of care from pre hospitalization through discharge. These activities include admission, continued, extended and discharge reviews in all reimbursement categories to determine medical necessity, assure high quality of care and efficient utilization of available healthcare resources, facilities and services. ESSENTIAL DUTIES AND RESPONSIBILITIES Is a role model for the Hospital's Patient Satisfaction effort when interacting with customers, subordinates and colleagues. Collaborates with the interdisciplinary team participants in team rounds to: facilitate timely care; sssure quality of care throughout the hospital stay; and minimize adverse outcomes. Initiates appropriate referrals to the internal interdisciplinary team. Communicates with Admitting or PFS regarding the needs of the patient, payer and provider are supported within the limitations of the existing individual benefit structure. Communicates relevant elements of the health plan benefits. Establishes a working diagnosis (DRG) on every patient at the time of admission to estimate the target length of stay or identify the date of discharge for planning and care coordination purposes. Communicates target LOS/estimated discharge readiness to physician, patient, family, care team and payor. Documents all team, physician and patient/family communication and concerns pertaining to coordination of care and services. Screens every patient to identify need for further assessment of medical necessity or discharge planning (standard description of which patients are seen). 11. Adheres to the Care Management Department policies and procedures. Participates in the Quality and Performance Improvement Plan for the Care Management Department. Considers the patient population served, age-specific criteria and the Watson Model of Care in all patient/family care and interaction. Reporting to the Director of Care Management, the Clinical Educator is accountable for the education program planning for all staff in the department. The Clinical Educator incorporates considerations of Age and Population Specific awareness and the Watson Model of Care in all program planning. The role is involved in the assessment, development and evaluation of staff competencies and considers compliance with regulatory and professional standards, improvement of quality, service levels, safety, customer satisfaction and collaborative relationships in the process. Serves as a consultant to the clinical areas and departments as it relates to curriculum development related to Care Management. Works with the department leadership team for new clinical initiatives and lead/support implementation in a positive and professional manner. Cultivates positive and collaborative relationships within the Care Management Department and with the nursing and clinical leadership team to ensure that educational needs are being met in the most effective manner possible. Provides leadership through educational activities to bring about changes in clinical practice. Conducts thorough chart auditing to ensure regulatory and quality components are done correctly. Performs regular audits of licensed staff per department schedule using standard audit tool(s); uses audit results to identify gaps in knowledge or compliance to policy and incorporates identified needs into curriculum and/or in services. Evaluates education programs on a regular basis and plans and implements improvements based on this evaluation and feedback from customers. Plans and implements the orientation of all new hires to include: general hospital orientation; clinical orientation; Compass module coordination, and EMR training as well as follow-up with orientees in the clinical area. Utilizes adult learning principles in program planning to select the teaching strategy and method appropriate for the learner and the content including e-learning. Ensures the curriculum utilizes the information generated from the Quality and Performance Improvement activities and MLKCH Care Management Policies and Procedures as a foundation and incorporates these components into the education provided to the staff. Coordinates the implementation of all education in the department; providing appropriate learning materials for the curriculum. Serves as primary liaison with Education department to ensure class room training is documented and entered into the Learning Management System; collaborates on content for development in self-paced eLearning modules. Maintains appropriate records for all education programs. Responsible for maintaining employee education files in the Education department and follow up on needed certifications. May be assigned design and implementation of special projects with hand off to appropriate parties. Serves as a department backup for Kronos time-keeping. Reporting to the EmPATH Manager, the EmPATH Registered Nurse has 12 hour accountability for organizing, planning, directing, coordinating, and providing high quality, individualized patient/family centered care based on the Watson Model of Care for a defined group of patients, including the identification and demonstration of abilities to meet special needs and considerations of Age and Population Specific awareness. Oversees direct patient care, performs patient assessments, Behavioral Health assessments, eligibility for 5150 designation and is responsible to ensure safety at all times.
