Patient Advocate & Community Liaison

Silver Spring Home Health & Hospice is seeking a compassionate, motivated, and relationship-driven Patient Advocate & Community Liaison to serve as the bridge between our patients, families, referral partners, and the community.

In this role, you will advocate for patients, address concerns, enhance patient satisfaction, build community partnerships, develop referral sources, and support business growth. The ideal candidate has healthcare sales experience, strong communication skills, and a passion for delivering exceptional customer service.

Qualifications:

Minimum 1 year of Home Health Sales experience (required)

Minimum 1 year of Healthcare Sales experience (required)

Strong relationship-building and networking skills

Excellent communication and problem-solving abilities

Knowledge of home health and hospice services preferred

Bilingual English/Spanish is a plus
