Home Health Registered Nurse (RN) – Macon, GA
Schedule: Monday–Friday, 8:00 AM–5:00 PM, with on-call rotation once every three weeks
Productivity: 30 units; hours are not guaranteed
Mileage: $0.43 per mile
Coverage Area: Bibb, Crawford, Monroe, Jones, and Twiggs counties
Job Summary
We are seeking an experienced Home Health RN with required HCHB/OASIS and home health experience to provide comprehensive patient assessments, coordinate care, and support positive patient outcomes.
Responsibilities
- Complete comprehensive patient assessments and develop individualized care plans.
- Provide wound care and IV care.
- Coordinate care with physicians, social workers, and other healthcare professionals.
- Manage care transitions and connect patients with appropriate medical and nonmedical resources.
- Monitor treatment compliance, identify potential risks, and promote positive outcomes.
- Maintain accurate clinical documentation while protecting patient confidentiality.
Requirements
- HCHB/OASIS experience and prior home health experience required.
- Active Georgia RN license or compact license.
- Wound care and IV care experience required.
- Associate or Bachelor’s degree in Nursing or a related field.
- Strong clinical judgment, assessment, and care coordination skills.
- Case management certification preferred.
