RN - Discharge Planner & Case Manager

This position works on-site at Memorial Hospital in the Nursing Department, Monday - Friday; 40-hours a week on day-shift. Job Summary The RN Discharge Planner / Case Manager coordinates and facilitates patient care transitions to ensure safe, timely, and appropriate discharge planning. This role collaborates with patients, families, physicians, nursing staff, and community resources to support continuity of care, optimize resource utilization, and promote positive patient outcomes while ensuring compliance with regulatory and payer requirements. The RN is the swing bed coordinator. The RN is also responsible for assisting with scheduling, staffing, and coverage of work areas when needed, including Supervisor responsibilities and expectations.

Qualifications
Education & Licensure:
• Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) required.
• Current Registered Nurse (RN) license in the State of Illinois required.
• Current BLS (CPR) certification required. Experience:
• Minimum of 2 years of clinical nursing experience required.
• Experience in case management, discharge planning, utilization review, or care coordination preferred.
• Supervisor or management experience preferred.
• Knowledge of acute care, post-acute services, and community resources preferred.

Immunizations/Health Requirements
• Vaccinations required by IDPH and CDC.
• TB testing
• Fit testing
