Concord Health at Home is seeking 3 experienced and compassionate Registered Nurses (RN) to provide one-on-one skilled nursing care for a medically complex patient in the home setting. This private duty home health role involves advanced nursing interventions, including PICC line management, Total Parenteral Nutrition (TPN), and home mechanical ventilator care. The RN will deliver patient-centered care independently while supporting the patient and family in the home environment. Full-time and part-time opportunities are available.

Responsibilities

• Provide skilled home health nursing care.
• Manage and maintain PICC lines.
• Administer and monitor TPN therapy.
• Assess and manage home mechanical ventilation.
• Perform patient assessments and monitor vital signs.
• Administer medications and treatments as ordered.
• Educate patients and families regarding care plans and equipment.
• Complete accurate clinical documentation.

Required Qualifications

• Associate Degree in Nursing.
• Current, unrestricted Registered Nurse (RN) license.
• Basic Life Support (BLS) certification.

Preferred Qualifications

• At least one year of registered nursing experience.
• Experience with PICC line care.
• Experience administering Total Parenteral Nutrition.
• Experience with mechanical ventilation.
• Experience with medication administration, patient assessment, clinical documentation, and patient education.
