JOB RESPONSIBILITIES
- Conduct patient assessments and develop individualized plans of care.
- Coordinate care with physicians, patients, families, and other healthcare professionals.
- Educate patients and caregivers on care plans, treatments, and ongoing health needs.
- Supervise and support LPNs, HHAs, and CNAs to promote quality patient care.
- Review and update plans of care as needed.
- Maintain accurate, timely, and complete clinical documentation.
- Participate in case conferences and agency quality improvement initiatives.
- Help ensure patients receive compassionate, safe, and effective care in their homes.
QUALIFICATIONS
- Graduate of an accredited nursing program.
- Active Indiana RN license or a qualifying multistate license recognized by Indiana.
- Minimum of 2 years of nursing experience; home health experience preferred.
- Current CPR certification.
- Strong communication, leadership, organizational, and clinical skills.
- A commitment to providing exceptional patient-centered care.
