Registered Nurse (RN) – Hospice Admissions

Salary: $85,000–$95,000/year + Benefits

Cell Phone: $60/month

We are seeking an experienced and compassionate Hospice Admissions RN to provide quality patient and family-centered care.

Responsibilities:

• Assess patients for hospice eligibility and complete admissions.
• Develop individualized care plans and provide skilled nursing care.
• Educate patients and families on hospice services and care plans.
• Collaborate with physicians and the interdisciplinary team.
• Maintain accurate clinical documentation.
• Participate in on-call rotation and travel within the assigned territory.

Requirements:

• Active Texas RN license.
• Minimum 1 year hospice or senior care experience.
• Strong clinical assessment and communication skills.
• Valid driver’s license, reliable transportation, and auto insurance.
• Background check and professional references required.

Schedule: Monday–Friday, 8:00 AM–5:00 PM + on-call rotation
