Pay: $90,000 – $100,000 per year

Job Type: Full-time | Field-based

You'll be the clinical leader your team relies on: guiding and mentoring clinicians, coordinating care with families, physicians, and other providers, and making sure every patient receives high-quality, compliant care. Responsibilities:
  • Lead and oversee an interdisciplinary team of nurses, aides, social workers, chaplains, bereavement coordinators, and volunteers
  • Provide clinicians with real-time guidance, mentoring, and clinical problem-solving support
  • Lead interdisciplinary group (IDG) meetings and ensure plans of care areaccurate, current, and well documented
  • Conduct clinical record reviews, case reviews, and home visits as needed to ensure quality care
  • Ensure compliance with Medicare hospice regulations andmaintain survey readiness
  • Manage staff scheduling while fostering a positive, collaborative team culture
  • Develop and update patient care policies and emergency preparedness procedures
  • Serve as thecentral point of communication between patients, families, physicians, other providers, and the care team
  • Maximize resourceutilization to support efficient, high-quality care
Qualifications:
  • Active Registered Nurse (RN) license in the state you work in
  • Proficiency with electronic medical records (EMR)
  • Leadership or management experience a major plus
  • Strong understanding of the hospice philosophy and end-of-life care
  • Strong organizational, communication, and critical-thinking skills
  • A compassionate, patient- and family-centered approach
  • Valid driver's license and reliable vehicle with current insurance
