We are seeking compassionate, dependable, and skilled Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide individualized Shift Nursing services to adults with intellectual and developmental disabilities in their homes and community settings.
Position Summary
The RN/LPN Shift Nurse provides one-on-one nursing care based on each individual's assessed medical needs, authorized healthcare orders, nursing plan of care, and Individual Support Plan (ISP).
Nurses provide services within their professional scope of practice while helping individuals maintain their health and safety and remain as independent as possible in their homes and communities.
Assignments and schedules vary according to each individual's authorized services and healthcare needs. Ability and willingness to travel throughout Philadelphia, Delaware, Montgomery, Bucks, Chester, and surrounding counties
Responsibilities
- Provide individualized nursing services within the RN or LPN's authorized scope of practice.
- Follow physician or authorized practitioner orders, nursing plans of care, Individual Support Plans (ISPs), and established healthcare protocols.
- Monitor and observe individuals for changes in health status, symptoms, or other medical concerns.
- Administer medications and treatments as ordered and permitted within professional scope of practice.
- Monitor medication effectiveness and observe for potential adverse reactions or changes in condition.
- Perform authorized nursing procedures and treatments based on the individual's assessed healthcare needs.
- Monitor vital signs and other health indicators as required by the individual's plan of care.
- Provide appropriate nursing interventions to maintain or restore health and well-being.
- Recognize and respond appropriately to changes in condition, urgent medical concerns, and emergencies.
- Communicate significant health changes or concerns to appropriate healthcare providers, agency leadership, family/support team members, and emergency services when required.
- Provide health education and instruction to individuals, families, caregivers, and support staff when appropriate.
- Encourage individuals to understand and participate in their healthcare to the greatest extent possible.
- Promote infection prevention, medication safety, and appropriate health and safety practices.
- Maintain accurate and timely nursing documentation, including nursing notes, medication and treatment records, observations, interventions, and other required records.
- Collaborate with physicians, healthcare professionals, Supports Coordinators, families, caregivers, and other members of the individual's support team as appropriate.
- Maintain confidentiality and comply with HIPAA, agency policies, professional nursing standards, and applicable state and ODP requirements.
- Complete all required agency, ODP, and individual-specific training.
- Maintain professional boundaries while providing compassionate, respectful, and person-centered care.
- Promote each individual's dignity, choice, independence, health, safety, and overall quality of life.
Qualifications
- Current and unrestricted Registered Nurse (RN) or Licensed Practical Nurse (LPN) license permitting practice in Pennsylvania.
- Graduate of an approved nursing education program.
- Experience providing nursing services to individuals with intellectual and developmental disabilities is preferred.
- Home and community-based nursing, private-duty nursing, behavioral health, or related nursing experience is preferred.
- Active National Provider Identifier (NPI) and Pennsylvania Medical Assistance Master Provider Index (MPI) number are strongly preferred.
- Candidates must be willing and able to complete any required Pennsylvania Medical Assistance/PROMISe enrollment, credentialing, or provider-association requirements necessary to provide services through A Fulfilling Life Services.
- Ability to understand and follow healthcare orders, nursing plans of care, and Individual Support Plans (ISPs).
- Strong clinical observation, communication, documentation, and decision-making skills appropriate to professional licensure.
- Compassionate, dependable, patient, and committed to person-centered care.
- Ability to work independently in home and community-based settings.
- Ability to successfully complete required criminal history and other applicable background clearances.
- Valid driver's license, reliable transportation, and current auto insurance.
- Ability and willingness to travel throughout Philadelphia, Delaware, Montgomery, Bucks, Chester, and surrounding counties based on individual assignments and service needs.
Training Requirements
- Complete all required ODP and A Fulfilling Life Services orientation and training requirements.
- Maintain all professional licenses, certifications, and required continuing education throughout the contracting relationship.
Technology & Documentation Requirements
- Basic computer, smartphone, and technology skills.
- Ability to download, navigate, and regularly use required mobile applications.
- Ability to use electronic systems for scheduling, timekeeping, service verification, nursing documentation, and other required work-related functions.
- Ability to complete accurate documentation within required timeframes.
NPI & MPI
Candidates who already have their own National Provider Identifier (NPI) and/or applicable Pennsylvania Medicaid Provider ID/MPI are strongly preferred. We will still consider candidates without these identifiers, depending on the assignment and enrollment requirements.
Scheduling
Schedules vary depending on the needs of individuals receiving services. Opportunities may include day, evening, overnight, weekday, and weekend shifts.
Availability, preferred days, and the distance you are willing to travel will be discussed during the interview process.
