Job Description:

Responsibilities:
- Conduct comprehensive patient assessments to determine rehabilitation needs.
- Develop and implement individualized care plans in collaboration with the healthcare team
- Administer medications and treatments as prescribed by physicians
- Monitor patient progress and adjust care plans accordingly to achieve rehabilitation goals.
- Educate patients and their families about treatment plans and self-care techniques
- Collaborate with physical therapists, occupational therapists, and other rehabilitation professionals to optimize patient outcomes
- Document patient care activities accurately and maintain up-to-date medical records
- Ensure compliance with health and safety regulations and protocols
- Assist in the discharge planning process to ensure continuity of care post-rehabilitation
- Stay updated on best practices and advancements in rehabilitation nursing care
Requirements/Qualifications:- Valid and current Registered Nurse (RN) license in the state of practice
- Bachelor of Science in Nursing (BSN) preferred
- Minimum of 2 years of nursing experience in a rehabilitation or related setting
- Certification in Rehabilitation Nursing (CRRN) preferred or willingness to obtain
- Basic Life Support (BLS) certification required
- Advanced Cardiovascular Life Support (ACLS) certification preferred
- Strong knowledge of rehabilitation nursing principles and practices
- Proficiency in electronic medical records (EMR) systems
- Excellent communication and interpersonal skills, Strong problem-solving and critical thinking skills
- Ability to work independently and as part of a multidisciplinary team
