Position Summary

The RN/LPN provides direct nursing care, medication management, and clinical support for residents (NRS 449) at Courage Worldwide’s Psychiatric Residential Treatment Facility (PRTF), Courage House Nevada, serving survivors of child sex trafficking and severe trauma. This role ensures delivery of trauma-informed, medically appropriate nursing services in compliance with Nevada licensing (NRS/NAC), Medicaid requirements, and CMS PRTF regulations (42 CFR Part 483 Subpart G).

The Registered Nurse (RN) practices within the full scope of professional nursing as defined by Nevada law, including assessment, care planning, clinical judgment, and supervision of nursing care.

The Licensed Practical Nurse (LPN) practices under the direction of an RN or licensed physician, providing nursing care consistent with Nevada scope of practice laws and facility policies.

This position integrates evidence-based nursing practice with trauma-informed, survivor-centered care, reflecting Courage Worldwide’s mission to restore dignity, safety, and long-term healing.

Qualifications

Registered Nurse (RN)

Current RN license in Nevada, in good standing

CPR/BLS certification (required)

Minimum 2+ years relevant experience

Licensed Practical Nurse (LPN)

Current LPN license in Nevada, in good standing

CPR/BLS certification (required)

Minimum 1+ year relevant experience

Nevada Scope of Practice Alignment

Registered Nurse (RN)

In accordance with NRS Chapter 632 and NAC 632, the RN:

Performs comprehensive nursing assessments and ongoing evaluation of health status Develops, implements, and modifies nursing care plans

Exercises independent clinical judgment in nursing care delivery

Delegates and supervises nursing tasks consistent with Nevada regulations

Provides patient education and care coordination

Administers medications and treatments as prescribed

Licensed Practical Nurse (LPN)

In accordance with NRS 632 and NAC 632, the LPN:

Provides nursing care under the direction of an RN or licensed physician Performs focused assessments and contributes to data collection

Implements established nursing care plans

Administers medications and treatments within scope and facility policy Reports changes in patient condition to the supervising RN or provider Does not independently perform comprehensive assessments or develop care plans

Core Responsibilities

1. Nursing Care & Clinical Support

RN Responsibilities:

Perform comprehensive nursing assessments and reassessments

Develop and oversee nursing care plans

Provide clinical oversight and supervision of LPNs and staff

LPN Responsibilities:

Deliver direct care based on established plans

Monitor and report changes in condition

Assist in implementation of nursing interventions

2. Medication Administration & Monitoring

RN:

Oversee medication administration systems

Evaluate effectiveness and safety of medications

LPN:

Administer medications per orders and scope

Document and report medication effects

3. Crisis Response & Safety

RN:

Lead clinical decision-making during crises

Provide assessment and intervention direction

LPN:

Assist in crisis response under supervision

Implement interventions as directed

PRTF Staffing & Coverage Requirements (Nevada + Federal Alignment)

To ensure compliance with CMS PRTF requirements (42 CFR 483.354) and Nevada licensing expectations:

Nursing Coverage Expectations

Nursing services must be available 24 hours per day, 7 days per week At least one licensed nurse (RN or LPN) must be on-site or immediately available at all times

RN Availability Requirements

An RN must be available for supervision of nursing services, either: On-site, or

On-call and able to respond within required timeframes

The RN is responsible for:

Oversight of nursing care delivery

Supervision of LPNs

Clinical decision-making and escalation

Staffing Ratios (Best Practice + Survey-Ready Language)

While Nevada PRTF regulations may not prescribe fixed ratios, Courage House Nevada maintains staffing consistent with acuity, safety, and CMS expectations, including:

Sufficient nursing staff to meet:

Resident medical and psychiatric needs

Medication administration schedules

Crisis response requirements

Increased staffing during:

High-acuity periods

Admissions and discharges

Medical or behavioral instability

Clinical Oversight Requirement

All LPN-provided care must occur under RN or physician supervision The RN ensures that care delivered meets:

Medical necessity

Safety standards

Regulatory compliance

Regulatory Compliance (Nevada + Federal)

Ensure compliance with:

Nevada Revised Statutes (NRS) Chapter 632 (Nursing Practice Act)

Nevada Administrative Code (NAC) Chapter 632

CMS PRTF regulations (42 CFR Part 483 Subpart G)

Medicaid documentation and billing requirements

PRTF Compliance Requirement

All duties of this position shall be performed in compliance with applicable Psychiatric Residential Treatment Facility (PRTF) regulations, federal and state laws, licensing requirements, accreditation standards, and Courage House policies and procedures.
