ESSENTIAL FUNCTIONS:
The Program Supervisor is responsible for the day-to-day management of all levels of operation for the Halfway or Tabitha House. The Supervisor will provide oversight for the health and safety of consumers and staff; and clinical leadership, direct supervision, evaluation and scheduling of SUD staff to ensure treatment, case management, educational and other recovery-oriented services are provided effectively to assist persons to live more independently and chemical-free within the community.
RESPONSIBILITIES:
- Provides training, coaching, and regularly scheduled supervision to all assigned direct-service staff and documents supervision, performance feedback, identified development needs, and follow-up actions in accordance with agency requirements.
- Conducts ongoing outreach and maintains regular contact with affiliated programs, agencies, courts, and other potential referral sources to generate qualified referrals and support achievement of established program census targets; documents outreach activities, contacts, referrals, and outcomes according to departmental requirements.
- Identifies and recommends appropriate agencies and community organizations for formal linkage agreements and monitors existing agreements at least annually to ensure they remain current, active, and aligned with program service needs.
- In collaboration with the Director of SUD, establishes measurable program outcome indicators and objectives and reviews progress at least quarterly to identify trends, performance gaps, and opportunities for improvement.
- Ensures that assigned consumers receive alcohol and other drug counseling, consumer education, and case management services consistent with their individualized treatment plans, program requirements, and applicable regulatory standards.
- Reviews consumer case records on a scheduled basis to ensure assessments, individualized treatment plans, treatment-plan reviews, progress notes, continuing-stay justifications, and discharge/aftercare plans are complete, accurate, timely, and consistent with HSI, CARF, DASA, and applicable regulatory requirements
- Reviews consumer files prior to closing or dismantling a program to verify that required documentation is complete and that records meet applicable HSI, CARF, and DASA requirements, correcting deficiencies before final closure whenever practicable.
- Ensures DARTS opening, closing, and service-reporting documentation is completed accurately for applicable services and submitted to the Billing/Data Entry Clerk within established submission deadlines.
- In collaboration with the Senior Residential Trainer, develops, reviews, and implements house-specific policies, procedures, daily activity schedules, and clinical intervention models for AOD residential facilities and evaluates their effectiveness as program needs change.
- Maintains oversight of assigned residential facilities to ensure they remain healthy, safe, adequately equipped, and conducive to consumer care; conducts periodic self-inspections, documents deficiencies, and follows up to ensure corrective actions are completed.
- Collaborates with internal departments and interdisciplinary team members to coordinate services, resolve operational or consumer-care issues, and support continuity of care for consumers with multiple service needs.
MINIMUM QUALIFICATIONS:
High school diplomas or GED.
Must have current IAODAPCA CADC certification, a minimum of five (5) years of experience in the AOD field and at least two years of continuous sobriety.
Documentation of three letters of recommendation from substance abuse professional staff upon hire.
Documentation of signed and dated acceptance of the Code of Ethics of the Association of Residential Extended Care Programs.
Must have good oral and written communication skills.
Computer Literate (i.e. proficient in Microsoft Word, Excel, Power Point).
