RN Case Manager

Timberland Health and Rehabilitation is seeking a compassionate and organized Registered Nurse (RN) Case Manager to join our skilled nursing team. This position plays an important role in coordinating resident care, supporting positive clinical outcomes, and ensuring effective communication among residents, families, providers, and the interdisciplinary care team.

• Active Registered Nurse (RN) license in Alabama

• Experience in long-term care, skilled nursing, or rehabilitation preferred

• Strong assessment, organization, and communication skills

• Ability to collaborate effectively with an interdisciplinary team

• Knowledge of care planning, discharge planning, and regulatory requirements

• Current CPR certification, or ability to obtain as required

• Weeknight on-call availability is required

• Weekend on-call availability is required

• Must be able to respond appropriately to clinical needs and facility concerns while on call

• Coordinate and monitor residents’ plans of care from admission through discharge

• Collaborate with nursing, therapy, social services, physicians, and other departments

• Communicate regularly with residents and families regarding care needs and progress

• Participate in interdisciplinary care plan meetings

• Assist with discharge planning and transitions of care

• Monitor changes in residents’ conditions and ensure appropriate follow-up

• Maintain accurate and timely clinical documentation

• Help ensure compliance with facility policies and state and federal regulations

• Advocate for residents and promote quality, resident-centered care

• Participate in the required weeknight and weekend on-call rotation
