Position Overview
The
RN Manager – Care Management will lead care management and case management operations, coordinating patient care across the healthcare continuum. This role works closely with patients, caregivers, nurses, physicians, social workers, payers, and other healthcare professionals to support effective transitions of care and improve patient outcomes.
Key Responsibilities
- Lead and oversee Care Management/Case Management operations and staff.
- Coordinate patient care and transitions across the healthcare continuum.
- Collaborate with physicians, nurses, social workers, payers, and other multidisciplinary teams.
- Address barriers to care, including psychosocial needs, mental health, substance use, abuse, and resource limitations.
- Support effective discharge planning and transitional care management.
- Promote patient-centered care and respect patient self-determination.
- Monitor care management performance, quality, and compliance.
- Provide leadership, coaching, and support to case management staff.
Required Qualifications
- Bachelor’s Degree in Nursing (BSN).
- Active California RN license upon hire.
- 5+ years of healthcare experience.
- 3+ years of Case Management experience in an acute care/inpatient hospital setting.
- 1+ year of healthcare leadership experience.
Preferred Qualifications
- Master’s degree in healthcare or related field.
- Certified Case Manager (CCM) certification.
- Accredited Case Manager (ACM) certification.
