Responsibilities
- Administer medications and treatments per physician orders, verify MARs, and document outcomes.
- Monitor residents’ vital signs, assess changes in condition, and report findings to the supervising nurse.
- Perform basic wound care, dressing changes, and support activities of daily living as needed.
- Maintain patient safety with fall precautions, infection control, and adherence to care plans.
- Collaborate with the RN and interdisciplinary team to coordinate patient care and communicate changes.
- Document nursing actions accurately in the electronic chart and ensure care plans are followed.
Requirements
- Active LPN license in good standing
- Ability to work evenings as scheduled
- Strong communication and teamwork skills
- Comfortable working in a long-term care setting and following facility policies
