We are seeking an experienced Registered Nurse (RN) Case Manager to provide compassionate hospice care, manage patient needs, coordinate services, and support patients and families throughout the hospice journey.
Responsibilities
- Perform comprehensive patient assessments and develop individualized care plans.
- Coordinate hospice services with physicians and the interdisciplinary care team.
- Administer medications and provide skilled nursing care.
- Monitor patient condition and communicate changes appropriately.
- Provide wound care, catheter care, and other nursing interventions as needed.
- Educate patients and families on medications, symptoms, and end-of-life care.
- Maintain accurate and timely clinical documentation.
- Coordinate follow-up care and support continuity of services.
- Travel within the assigned service area to visit patients.
Requirements
- Active RN license in good standing.
- Minimum 1 year of hospice experience required.
- Strong understanding of hospice philosophy and patient-centered care.
- Excellent communication and care-coordination skills.
- Ability to work independently and manage multiple patients.
- Valid driver’s license, reliable transportation, and ability to travel locally.
