Registered Nurse (RN) – Case Management

Location: Greer, SC 29651 | Work Setting:Hybrid

We are seeking a Registered Nurse (RN) with hospice experience to provide quality patient care and support case management and care coordination.

Responsibilities:

• Perform patient assessments and administer medications.
• Coordinate care, discharge planning, and follow-up services.
• Maintain accurate EHR/EMR documentation.
• Educate patients and families.
• Collaborate with interdisciplinary healthcare teams.
• Follow infection control and HIPAA standards.

Requirements:

• Active RN license
• 1+ year hospice care experience required
• BLS, ACLS, or PALS certification
• EHR/EMR experience preferred
• Strong communication and clinical skills
