Decision Critical Concierge Nursing agency is seeking exceptional Registered Nurses to provide personalized, one-on-one nursing care in the comfort of a patient’s home.
Founded and led by a Registered Nurse, Decision Critical is a concierge nursing organization dedicated to bridging the gap between the hospital, physician’s office, and home. Our nurses provide highly personalized clinical support during some of life’s most critical healthcare momentsfrom post-operative recovery and complex medical needs to chronic illness, elderly care, and end-of-life support.
Key Responsibilities
- Supports nursing care standards through adherence to existing policies and procedures of the Louisiana State Board of Nursing and Decision Critical.
- Obtains and documents nursing history from patients and/or significant others that is comprehensive of a patient’s present condition and past medical history. Performs basic assessment of patients, indicating adequate knowledge of anatomy, physiology, and pathology. Records vital signs, notes changes, and pursues more-specific investigations as needed. Identifies abnormal diagnostic data and shows a fundamental understanding of its implications by reporting information to the physician and others caring for the patient, and by incorporating information in the nursing care plan.
- Provides in-home nursing for patients as well as arranges services like transportation and therapy and seeks out third party resources that are beneficial for patients.
- Uses assessment information to develop a care plan before the end of a shift that communicates enough information for incoming staff to adequately care for the patient. Reviews and develops care plans incorporating physician orders for assigned patients. Includes the patient and his or her family in planning care, establishing priorities, and setting goals.
- Uses care plans as a basis for providing safe and therapeutic care to patients. Communicates with the patient and his or her family and other members of the health care team in carrying out the goals of the care plan. Accepts accountability for clinical care of assigned patients.
- Evaluates and notes the patient's response to nursing care defined in the nursing care plan and medical records.
- Assumes responsibility for professional growth by current reading and attendance at educational programs and staff meetings.
- Any other duties assigned by the administrative staff.
Physical Requirements:
- Prolonged periods of standing, bending, and reaching
- Must possess the physical ability to lift up to 50 pounds at times.
- Must possess the physical ability to perform tasks such as repositioning adult, bedridden patients without assistance.
- Must possess the physical ability to perform tasks such as assisting partially mobile adult patients with transfers to and from bed to wheelchair/walker and/or bathing.
- Must have own transportation to all worksites including, but not limited to, any Decision Critical assigned patient’s home or like facility.
- Must possess a smartphone with the ability and capacity to manage a HIPPA-approved medical app.
- Some travel may be required within a 60-100 mile radius of East Baton Rouge Parish.

Education and Experience:
- Candidate must have a current license as a Louisiana Registered Nurse.
- Minimum of one year of recent experience as a Registered Nurse.
- Have at least 2 professional references.
- Home healthcare experience is preferred.
