RN Clinical Manager, Home Health I Bonus Package

Compensation: $40–$45/hour based on experience, plus bonus

Aim Home Health has proudly served patients throughout the Chicagoland area for more than 15 years. We are seeking an experienced RN Clinical Manager to join our team in a full-time clinical leadership role overseeing patient care coordination, clinical operations, and Medicare home health case management.

This is an in-office position based at our Lisle office. The Clinical Manager is expected to work in the office at least four days per week, with one consistent weekday off.

The Clinical Manager works closely with our Director of Nursing, office team, and field clinicians to help ensure patients receive timely, coordinated, high-quality care while supporting the day-to-day clinical operations of the agency.

This role is ideal for an experienced home health RN who enjoys clinical oversight, problem-solving, supporting clinicians, and coordinating patient care and is looking for a more structured and supportive environment.

We are looking for someone who is:

• Organized and dependable
• Clinically strong with excellent judgment
• Confident managing patient cases from admission through discharge
• Experienced in home health clinical management
• Knowledgeable in Medicare home health processes and regulations
• Familiar with OASIS documentation and compliance standards

Primary Responsibilities:

• Manage assigned patient episodes from Start of Care through discharge
• Review and maintain plans of care, physician orders, and visit frequencies
• Review OASIS documentation for accuracy, completeness, and compliance
• Coordinate care with RNs, LPNs, PT/OT/ST, MSWs, and HHAs
• Communicate with physicians regarding orders, recertifications, and changes in condition
• Monitor patient progress, goals, labs, skilled need, and discharge planning
• Ensure compliance with Medicare home health regulations and agency standards
• Participate in case conferences and quality improvement initiatives
• Support coordination between field clinicians and office operations

Qualifications:

• Active Illinois RN license in good standing
• Minimum 2 years nursing experience required
• Home health case management experience strongly preferred
• Strong understanding of Medicare home health regulations and OASIS documentation
• Excellent communication, organizational, and clinical judgment skills
• Ability to manage multiple patient cases effectively and independently
• EMR/documentation proficiency required
