Key Responsibilities
Resident Care Coordination: Assess resident needs, develop and implement individualized care plans, and coordinate with supervising physicians to ensure optimal clinical outcomes and smooth recovery processes.
Shift Leadership: Oversee weekend 2nd shift nursing staff, ensuring all care delivery meets local, state, and federal standards. Monitor medication administration, treatments, and core clinical procedures.
Unit Rounds & Quality Assurance: Conduct regular rounds across assigned units, monitor treatment progress, and maintain accurate documentation in full compliance with HIPAA and facility standards.
Education & Support: Educate nursing staff, residents, and family members on clinical best practices, wellness, and care plans.
Team Collaboration: Work independently and collaboratively with facility leadership, assisting with clinical operations as directed by the Director of Nursing (DON), Assistant Director of Nursing (ADON), or Administrator.
Qualifications
Licensure: Active Registered Nurse (RN) or License Practical Nurse (LPN) license in good standing.
Experience: Prior supervisory experience in a long-term care, skilled nursing, hospital, or sub-acute facility required. Background in palliative or inpatient clinical care highly preferred.
Skills: Strong clinical assessment, team leadership, crisis management, and computer skills. Excellent verbal and written communication abilities.
