As a Case Manager, you will:
• Coordinate services for children and adults enrolled in the CHOICES Waiver and Community Transition Services (CTS) programs
• Facilitate development and annual review of Individual Service Plans (ISPs) using person-centered planning practices
• Assist individuals and families in accessing medical, behavioral health, educational, employment, housing, transportation, recreational, legal, and advocacy resources
• Monitor service delivery, document outcomes, and intervene as needed to ensure individuals' needs are met
• Develop collaborative relationships with providers, community agencies, families, and stakeholders
• Report and review Critical Incident Reports (CIRs) in accordance with program requirements
• Support Human Rights Committee (HRC) and Behavioral Intervention Committee (BIC) processes when appropriate
• Maintain timely and accurate documentation, reports, and service records
• Promote Personal Outcomes and the Charting the LifeCourse framework in service delivery
• Travel throughout assigned case coverage areas within South Dakota to support individuals and conduct required visits

• Bachelor's degree and one (1) year of related experience preferred, or an equivalent combination of education and experience
• Intermediate computer skills and ability to effectively utilize electronic documentation systems
• Ability to establish and maintain a HIPAA-compliant home office environment
• Valid driver's license, reliable transportation, and ability to travel throughout the assigned service area
• Ability to pass state and federal background checks and drug screening
• Experience working with individuals with intellectual and developmental disabilities
• Knowledge of Person-Centered Thinking, Person-Centered Planning, and Charting the LifeCourse principles
• Experience in case management, care coordination, social services, behavioral health, or human services settings
• Full-Time, 40 Hours Per Week
• Remote / Work From Home Position
• Flexible schedule based on caseload and service needs
• In-state travel required throughout assigned South Dakota coverage areas
• Occasional evening appointments may be necessary to meet individual and family needs
• Ability to sit, stand, walk, bend, stoop, reach, and drive for extended periods as required
• Ability to lift and carry up to 20 pounds occasionally
• Ability to navigate community settings, offices, homes, and uneven surfaces
• Ability to perform essential job functions with or without reasonable accommodation
