Job Description:\n\nJob Title: RN CHARGE\nWork EVERY MON; TUES; WED; THURS;\n64 HOURS BI-WEEKLY- GET PAID for 80 HOURS!!!\n2 BONUS DAYS!!\nDepartment: Nursing\nReports To: Director of Nursing\nFLSA Status: Non-Exempt\nRevised Date: September 3, 2026\nSUMMARY\nSupervises and coordinates nursing activities in the facility by performing the following duties, which includes both direct and in-direct care. Nurse shall perform routine procedures such as obtaining and recording of vital signs, administering medications as well as providing specialized treatments for progressive and chronic conditions. Nurse shall also provide educational and emotional assistance to resident and resident representative.\n.\nESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.\nAssigns duties and coordinates nursing service\nEvaluates nursing activities to ensure resident care, staff relations, and efficiency of service\nObserves nursing care and visits residents to ensure that nursing care is carried out as directed, and treatment administered in accordance with Physician’s instructions\nPrepares and assists residents for eating and drinking during meals, transports residents to dining room, assures food is accessible, feeds residents requiring assistance, prompts residents requiring cueing, supervises self-feeding residents, cleans residents after meals with face washing, hand washing, clothing, trays, wheelchairs, tables, passes trays, etc.,\nMakes sure all residents are offered nourishment and hydration at meals and during the shift\nDirects preparation and maintenance of residents’ clinical records\nReviews physician orders to ensure accuracy of MAR, TAR and other documentation\nReviews transfer sheet, medications, diet and MD orders; obtain other orders as necessary (e.g. therapy, podiatry consult, etc.)\nCompletes head to toe evaluation of new admissions/readmissions for completion of the following UDAs: Admission, Braden, Fall, Wander and Pain evaluation and other evaluations, such as SBAR, etc.\nServes as a role model in the delivery of direct nursing care. Provide guidance, support and constructive feedback to nursing personnel\nAssists in the transfers and lifts of residents as needed; walks ambulatory residents as needed.\nInspects rooms and resident care areas for cleanliness and comfort\nAccompanies Physician on rounds and keeps informed of special orders concerning residents\nResponsible for recognizing and resolving resident/family concerns in a prompt and professional manner. Identifies and initiates when interdisciplinary support is needed.\nCompletes appropriate sections of UDA for incident/accident reports and assesses resident for injuries via completion of body audit and obtains/records vital signs, as indicated, such as temperature, pulse, blood pressure, and other vital signs\nAnswers call lights, personal alarms and residents’ needs and responds to the resident request, for assistance with ADL’s, such as toileting, repositioning/boosting, hydration, transfers and socialization, etc.\nParticipates in orientation and training of personnel\nResponsible to manage and maintain the PPD and staffing ratio after hours and on weekends/holidays to ensure both Per Patient Day and staff to resident ratio achieved.\nOrders/directs ordering of drugs, solutions and equipment and maintains records on narcotics\nInvestigates and resolves concerns/complaints and completes concern form along with corrective action/response; refers unusual problems to superior\nObtains report from off going shift; presents report to on coming shift. Reports all changes in conditions etc. to the Nursing Supervisor/designee.\nIdentifies safety hazards; initiates corrective action; initiates immediate action in emergency situation involving resident conditions\nFollows residents rights program and abuse prevention protocol\nEncourages nursing staff to perform to their fullest potential by creating a culture of learning and support\nParticipates in educational activity and coordinates activity with Director of Staff Development/DON\nAttends staff meetings, participates in employee disciplinary measures, assumes responsibility for compliance with all appropriate rules, regulations, etc. Implements the progressive disciplinary process when necessary, recommends or initiates disciplinary action based on facility or nursing policy and procedures.\nCompletes reports as assigned, developed care plans, administers and documents medication\nEnsures pertinent medical information needed to care for resident is located in resident plan of care, to include person-centered interventions\nAccurately and objectively document pertinent resident information in the clinical record following the facilities policies and procedures regarding documentation\nPerforms IV insertions, administration of IV medications/ fluids for those with PICC or midlines, or other RN specific clinical duties, ie portacath care, removal of PICC lines/ midlines.\nAssist med nurses with procedures as needed - catheters, bladder scans\nFollows facility admissions policy and facility discharge and transfer policy\nParticipates in resident care plan meetings as requested\nCounts all narcotics in accordance with established facility policy\nSupervises and supports resident care, prepares and administers tube feeding, medications etc.\nNotifies the physician of changes in condition, labs, x-rays, etc.\nPerforms resident care as per other duties as assigned and as indicated in job duties\nInitiates and participates in Cardiopulmonary Resuscitation\nMaintain resident confidentiality at all times\nParticipates and supports Quality Assurance Performance Initiatives and cultural changes through Performance Improvement Project teams, etc\nParticipates in all required in service education.\nSUPERVISORY RESPONSIBILITIES\nDirectly supervises the med-treatment nurses and the nursing assistants. Carries out supervisory responsibilities in accordance with the organization’s policies and applicable laws. Determines work procedures, directs and expedites workflow, trains, addresses complaints and resolves problems.\nQUALIFICATIONS\nTo perform this job successfully, an individual must be able to perform each essential duty satisfactorily. A minimum of one year’s experience in related nursing positions is preferred.\nLANGUAGE SKILLS\nAbility to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Proficient interpersonal relations and communicative skills\nMATHEMATICAL SKILLS\nAbility to apply concepts such as fractions, percentages, ratios and proportions to practical situations and consistently and accurately calculate drug dosages.\nREASONING ABILITY\nAbility to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
