About the Role:

Infinity Love Home Care Agency is seeking a highly organized, mission-aligned Case Manager to oversee patient care coordination and caregiver management. The ideal candidate is a proactive professional with a deep understanding of the home care landscape and a commitment to maintaining the highest standards of clinical and administrative compliance.

Preferred Qualifications:

  • Experience:Minimumof2 years ofdirectexperienceinHomeCareCaseManagement.
  • TechnicalSkills:ProficiencyinHHA Exchange isstrictlyrequired.
  • Knowledge:Deepunderstandingofhomecareregulations,Medicaidwaivers,andEVV (Electronic Visit Verification) requirements.
  • Credentialing:Mustbefullycredentialedandabletopassarigorousbackgroundcheck and child abuseclearance.
  • Attributes:Strongcommunicationskills,theabilitytomulti-taskinafast-pacedenvironment,and a "patient-first"mentality.

Responsibilities:

  • Caseload Management: Overseeadedicatedcaseloadofpatients,ensuringallcareplans arefollowedand updated inaccordance withstate regulations.
  • HHA Exchange Oversight: Managedailyscheduling,authorizationtracking,and compliancedocumentationwithin theHHA Exchange platform.
  • Caregiver Coordination: ActastheprimarypointofcontactforHomeHealthAides (HHAs);managecaregiver schedules,performance,and credentialingupdates.
  • Compliance & Quality Assurance: Ensureallpatientfilesandcaregivercredentialsare fully"audit-ready" and compliant withPA Departmentof Health standards.
  • Collaboration:Workcloselywiththeclinicaldirectorandintakedepartmenttotransition new clientssmoothlyintoservice.

Skills:

Strong communication, empathy, and advocacy to connect with clients, plus sharp organizational, time management, and problem-solving skills to coordinate care, resources, and documentation effectively, all while showing cultural competence and resourcefulness to tailor support to individual needs. Key skills include building trust, assessing needs, creating care plans, coordinating services (like personal care, errands), and managing complex client situations with sensitivity.
