Position Summary

Greek American Rehab & Care Center is seeking a dedicated and compassionate Full-Time Occupational Therapist (OT) to join our rehabilitation team. The Occupational Therapist will provide high-quality evaluation, treatment, and rehabilitation services to residents with the goal of improving independence, functional abilities, safety, and quality of life.

Essential Responsibilities
  • Evaluate residents' functional abilities and develop individualized occupational therapy treatment plans.

  • Provide skilled occupational therapy interventions based on each resident's needs and established plan of care.

  • Address activities of daily living, mobility, coordination, strength, balance, cognition, and other functional skills.

  • Monitor residents' progress and modify treatment plans as appropriate.

  • Document evaluations, treatments, progress, and discharge information accurately and timely.

  • Collaborate with physicians, nurses, physical therapists, speech therapists, nursing assistants, social services, and other members of the interdisciplinary team.

  • Educate residents, families, and caregivers regarding treatment programs, adaptive techniques, and safety strategies.

  • Maintain compliance with applicable federal, state, and facility regulations and professional standards.

  • Participate in care planning and interdisciplinary meetings as needed.

  • Promote a safe, respectful, and supportive environment for all residents.

Qualifications
  • Current Occupational Therapist license in the State of Illinois.

  • Graduate of an accredited Occupational Therapy program.

  • Current CPR certification, if required by facility policy.

  • Experience in skilled nursing, rehabilitation, long-term care, or a related healthcare setting preferred.

  • Strong communication, organization, documentation, and interpersonal skills.

  • Ability to work effectively as part of an interdisciplinary healthcare team.

  • Compassionate, dependable, and committed to providing resident-centered care.
