Job Summary:

As a PRN-Social Worker, you will provide a wide variety of services to patients, caregivers, and families, identifying and addressing emotional and psychosocial needs in advanced illness at end of life. Your expertise will guide families through emotional, psychological, and practical challenges, offering vital support during a crucial time.

Responsibilities:

  • Conduct assessments of psychosocial status, communicating findings to the Registered Nurse and interdisciplinary team members to inform care plans

  • Conduct safety and environment assessments, identifying and developing alternative plans to ensure patient well-being

  • Perform social evaluations and plan interventions to address patient and family needs

  • Provide counseling to patients and families, focusing on stress management and coping with difficulties related to the illness

  • Maintain accurate clinical records and enter data into the Electronic Medical Record (EMR) system at the bedside

  • Offer information and referral services to patients and families regarding practical and environmental needs

  • Serve as a liaison between patients, families, and community agencies to facilitate access to services

  • Develop and maintain contacts with public and private agencies to enhance resources available for patients and staff

Requirements:

  • Bachelor of Science in Social Work, Psychology, or Sociology; MSW preferred

  • Minimum of one year's experience in healthcare, with hospice experience preferred

  • Demonstrates good verbal and written communication, and organizational skills
