Advanced Practice Provider - LTC

Nurse Practitioner / Physician Assistant – Skilled Nursing & Long-Term Care

Location: Eugene, OR
We are seeking a compassionate Nurse Practitioner (NP/ARNP) or Physician Assistant (PA) to provide medical care in Skilled Nursing and Long-Term Care settings.

Responsibilities

• Assess, diagnose, and treat geriatric and post-acute patients
• Develop and manage treatment plans
• Prescribe medications and interpret labs/tests
• Provide post-hospital follow-up
• Document care in the EMR
• Collaborate with physicians and interdisciplinary teams
• Ensure regulatory and quality compliance

Requirements

• Graduate of an accredited NP/ARNP or PA program
• Current Oregon license and national certification
• SNF/LTC, geriatrics, post-acute, or primary care experience preferred
• Strong clinical, communication, and organizational skills
