Home Care Coordinator

The Home Care Coordinator manages and organizes all aspects of healthcare coordination for a small caseload of up to 30 patients. This role involves coordinating care plans, educating patients on available resources, and facilitating communication among patients, families, and healthcare teams. The coordinator ensures quality care while maintaining patient confidentiality and supports a multidisciplinary approach to optimize patient health outcomes.

Responsibilities

• Creates schedules and organize clients appointments and treatments
• Educate clients on healthcare resources and management options
• Coordinate communication between healthcare staff, patients, and families regarding day to day changes
• Maintains call in and call outs of caregivers
• Recruit, train, and schedule staff to support patient care
• Develop patient care goals and monitor progress
• Maintain patient confidentiality and uphold quality care standards
• Perform Supervisory visits as needed
• Answers caregiver concerns following Hands with Heart Home Healthcare Policies and procedures
• Open and Close the office
• Paid time off after 1 year of service

Preferred Qualifications

• 1+ years of experience in healthcare coordination
