Home Health Nurse

for the

Palace Suites

Independent Living Community

The #1 Priority of this position is

To coordinate the care of our patients and assist with any Medicare issues.

Essential Duties & Responsibilities

  • Review all admission documentationin order to ensure the quality of the services to be provided.
  • Responsible for coordination withall field staff, that they follow agency’s policies and procedures.
  • Responsible for reviewing thepatient’s plan of care and ensuring that it gets properly implemented,including discussing it with the patients as necessary.
  • Maintains daily communication withfield staff in order to ensure that all services are provided as perpatient’s expectations.
  • Responsible for reviewing allphysicians’ orders obtained by field staff.
  • Verifies the proper delivery ofdaily services as scheduled, including unscheduled emergent visits inassigned community.

Qualifications

  • RN License required.
  • Home Health experience of knowledgeof Clinical Terminology preferred.

  • Responsible, always on time,resourceful, very accurate and detail-oriented.
  • Flexible as to work schedule. Mustbe available to work on weekends, holidays and in emergency situations.
