Personal Care Assistant Job Description
General Functions: Personal Care Assistant is responsible for care as directed by the RN Supervisor A. Implementation 1. Assist the RN Supervisor in prioritizing family needs and responding appropriately to those needs. 2. Follow the plan of Care as written by the RN Supervisor. 3. Perform all procedures safely. 4. Documents actions and client response to care provided. 5. Actively listens to any concerns expressed by the client. 6. Practice universal / standard precautions: a. proper hand-washing b. contaminated laundry 7. Use equipment safely 8. Maintain clients’ comfort and well-being. B. Duties 1. Provide patient supervision and companionship: 2. Meal Preparation. 3. Light house keeping. 4. Mobility assistance 5. Assess and report change in condition status. 6. Performs other duties as assigned. C. Reporting 1. Reports to the RN Supervisor the client’s response to care provided. 2. Assist the RN in revising the plan of care by objectively documenting and reporting client responses. 3. Assist the RN in revising short- and long-term goals for the client. 4. Assist and encourage the client in expressing any concerns or questions they may have. Responsible to: RN Supervisor Qualifications: 1. Must be 18 years of age. 2. Current CPR and First Aid Certification 3. 1-year experience in the hospital, nursing home, or home care environment. Relevant experience. 4. HS Diploma or equivalent. 5. Three references: two professional, one character. 6. Criminal background check. 7. Health Clearance- Annual Physical exam, PPD or chest x-ray, and Hep B clearance.
