Shift: 2nd shift 6:00pm-6:00am
Job Description
Be responsible for the care and well-being of the residents in your charge
Assess the residents’ needs and assist in developing and following individual care plans
Assure quality care is delivered through the utilization of nursing processes and guidelines
Identify and communicate changes in residents’ status to physicians, staff, and family members
Administer medications, treatments, and other direct care as prescribed and indicated
Coordinate admissions, transfers, and discharges
Communicate appropriate information to all staff to provide a smooth shift transition
Investigate and document incidents and accidents as required
Document appropriately as needed or required in PCC.

·Competitive salary RN $40-$42 an hour/ LPN $34-$36 an hour
1st shift differential .50 an hour
2nd shift differential $1.00 an hour
Weekend differential $2.00 an hour

Qualification
Current Indiana RN or LPN License
Current CPR certification
Experience working in a treatment center
