What You’ll Do:
- Provide Therapy Services and management services under established plans of care
- Treat patients in SNF and/or Outpatient settingsas appropriate
- Collaborate closely with an interdisciplinary team
- Deliver individualized, meaningful patient care
- Complete documentation efficiently and accurately
What We’re Looking For:
- VT Licensed PT, PTA, OT or OTA
