Hospice RN Case Manager

The Hospice RN Case Manager plays a vital role in delivering compassionate and comprehensive care to patients in a hospice setting. This role involves thorough patient assessments, developing and implementing individualized care plans, managing medications, and addressing symptoms to ensure comfort and quality of life. The RN also advocates for patients’ needs, supports families through the care process, collaborates with interdisciplinary teams, and maintains accurate documentation in compliance with healthcare standards.

Responsibilities

• Conduct patient assessments to evaluate physical and emotional needs
• Develop and implement personalized care plans
• Manage and monitor medications to ensure patient safety and comfort
• Provide symptom management to alleviate discomfort
• Advocate for patients’ rights and care preferences
• Support families through education and emotional guidance
• Coordinate care with interdisciplinary healthcare team members
• Document all patient interactions and care activities accurately
• Ensure compliance with hospice and healthcare regulations

Preferred Qualifications

• Entry-level experience in hospice nursing
