TITLE:Case Manager, Behavioral Health RESPONSIBLE TO:Regional Leadership WORKAREA:Outpatient sites and Community

SUMMARY:The Case Managers primary role, under the direction of Regional Leadership and the Case Manager Supervisor, is to provide support and coordinating services based on client needs and treatment plans. The case manager will actively engage in a professional, collaborative process to support our clients' mental and physical health needs. As a Case Manager, you will assess, plan, implement,coordinate,monitor,advocate,andevaluatetheoptionsandservicesrequiredtopromote health, quality, and cost-effective outcomes for our clients, ensuring patient safety and adherence to quality care standards.

Inthisrole,youwillworkcloselywithothercareteammemberstoplan,link,advocate,coordinate, and monitor client care. This collaboration will help clients access a wide range of resources, including but not limited to health services, financial assistance, housing, employment, education, and social services. The Case Manager is also responsible for successfully managing their caseload and communicating with the treatment team as it relates to Case Management across various disciplines.

DUTIESANDRESPONSIBILITIES Day-To-Day Responsibilities
  • Actasaliaisonbetweenthetherapist,othercareteammembers,client,andclient'sfamily to coordinate the best care.
  • Arrangefamilyvisits,coordinatetransportation,andensureclients'needsaremet.
  • Maintainclearandongoingcommunicationbetweenthefamily,referralsource,andPath Behavioral Healthcare.
  • Attendcourthearingsasappropriate.
  • Ensuredocumentationandbillingadheretopolicy,PathBehavioralHealthcarestandardsof practice, and all applicable compliance standards within the EHR system.
  • Maintaincompliancewithchart/documentationauditcriteria,billingreviews,andPhase System of Care requirements.
  • Attendsupervisoryandteammeetings;presentaccuratecasemanagementreportsreflecting services provided and clients' responses to treatment.
  • Facilitateindividualandgroupmeetingsasassigned.
  • Serveonagency qualityassurance orothercommitteesasassigned.
  • Facilitatethefurtherdevelopmentofdailylivingskillsifidentifiedbytheclientand/or parent or guardian.

  • Addressandresolveclient/guardianconcernsinatimelymanner.
  • Oversee the implementation of the individual plan of service, supporting the client's aspirations,goals,anddesiresforoptimizingindependence,promotingrecovery,and developing and maintaining natural support systems.
  • Ensureongoingclientparticipationindiscussionsoftheirplans,goals,andstatus.
  • Identifyandaddressgapsinservice,aswellasmonitorunderandover-utilizationof authorized services.
  • Identifyandassistinbusinessdevelopment/referralsourcecommunicationandcollaboration efforts.
  • Coordinateandassisttheclientincrisisinterventionanddischargeplanning,including community support systems following hospitalization.
  • Maintainanorganizedpracticeandschedule,beatworkontimeandreadytowork,and leave work at the completion of client care responsibilities.
  • Reduceclientreschedulingbyinformingtheappropriatepersonoftimeoffwork.
  • MaintainstrictcompliancewithHIPAAguidelines,confidentialityagreements,billingand coding guidelines, and company policy.
  • Maintainademeanorofpositiveprofessionalismandserveasamodelforothersto exemplify the mission and vision of Path Behavioral Healthcare.

  • Mentalhealthinterventionsaddressingsymptoms,behaviors,thoughtprocesses,etc.,tohelp clients eliminate barriers to seeking or maintaining education and employment.
  • Activitiesthatincreasetheclient'scapacitytopositivelyimpacttheirenvironment.
  • AssumeadditionaldutiesasassignedbytheCaseManagerSupervisororRegional Leadership.

QUALIFICATIONS

Education:CandidatemusthaveaHighSchoolDiplomaorGED.Bachelor’sdegreeispreferred.

Experience:Candidatesmusthaveaminimumof3yearsofcasemanagementexperience.

PhysicalEffort:Thispositionrequirestheabilitytoworkunderstressfulconditionsandtowork irregular hours.

CommunicationSkills:Candidatesmustpossessexcellentverbalandwrittencommunicationskills in order to communicate professionally via telephone and in writing. Strong grammar and writing skills are crucial for clear and concise communication with clients, colleagues, and other healthcare professionals.

InterpersonalSkills:Thesuccessfulcandidatemustdemonstratetheabilitytointeractandassistin a friendly, compassionate, and professional manner with colleagues, providers, clients, and their families, as they represent both themselves and Path Behavioral Healthcare.

Essential Technical/Motor Skills:Candidates must achieve competency in the Electronic Health Record(EHR)program,telemedicineservices,andanyadditionalsoftwareprogramsutilizedbythe company, ensuring efficient access and updating of client information. The role requires fine dexterity, adept handling, and proficient gripping abilities to perform various tasks.

Background:The candidate must pass all federal and state background checks, including a clean Motor Vehicle Record. Educational and work history will be confirmed in compliance with company policies.
