Position Overview
The Hospice RN Case Manager is responsible for coordinating and delivering comprehensive nursing care to hospice patients.
This role supports patients and families throughout the end-of-life process.
Key Responsibilities
Perform nursing assessments
Develop and manage care plans
Administer medications and treatments
Educate patients and families
Coordinate interdisciplinary care
Monitor patient condition
Maintain accurate documentation
Participate in on-call rotation
Qualifications
Active RN license in Massachusetts
1–2 years nursing experience preferred
Strong clinical and communication skills
Ability to work independently
Reliable transportation
