Occupational Therapist (OT) – 3 Facilities | Rochester, NY | Full-Time, Part-Time & PRN
Stern at Home Therapy is hiring Full-Time, Part-Time, and PRN Occupational Therapists (OTs) to provide one-on-one therapy services across three established facilities in the Rochester, NY area.
This is an excellent opportunity for an Occupational Therapist looking for the consistency of facility-based care without the extensive driving associated with traditional home health. Your caseload will be concentrated across three established facilities, allowing you to maintain a steady patient volume while limiting unnecessary travel.

What You'll Do:
As an Occupational Therapist with Stern at Home Therapy, you'll evaluate and treat a primarily geriatric patient population across three established facilities in the Rochester area.
You'll maintain clinical autonomy over evaluations and individualized treatment plans while working alongside facility staff and Stern's administrative and clinical teams.
Responsibilities include:
- Perform occupational therapy evaluations and develop individualized plans of care.
- Provide one-on-one occupational therapy treatments across three established facilities.
- Help patients improve independence with activities of daily living.
- Address upper-extremity strength, range of motion, coordination, cognition, safety, and functional performance.
- Provide therapeutic activities and exercises based on each patient's individual goals.
- Recommend adaptive equipment and strategies when appropriate.
- Educate patients and caregivers on adaptive techniques, exercises, and safety.
- Monitor patient progress and modify treatment plans as clinically appropriate.
- Complete accurate and timely clinical documentation.
- Communicate with physicians, caregivers, families, facility staff, and interdisciplinary care team members.
- Coordinate with Stern's administrative team regarding scheduling and patient care.
Compensation:
Full-Time – Minimum 16 Patients:
Full-Time W-2:
- $45–$50 per visit
- Facility-based compensation
- Consistent caseload across three facilities
Full-Time 1099:
- $55 per visit
- Facility-based compensation
- Consistent caseload across three facilities
Part-Time – Minimum 10 Patients:
Part-Time W-2:
- $45–$50 per visit
- Fixed per-visit compensation
Part-Time 1099:
- $55 per visit
- Fixed per-visit compensation
PRN 1099 – Minimum 5 Patients:
- $55 per visit.
- Fixed per-visit compensation.
- PRN positions are 1099 independent contractor roles only.

Qualifications:
- Active Occupational Therapist license in New York required.
- Graduate of an accredited Occupational Therapy program.
- Reliable transportation for travel between the three facilities.
- Strong clinical, communication, documentation, and time-management skills.
- Ability to collaborate effectively with facility staff and interdisciplinary care teams.
- Geriatric or facility-based therapy experience preferred but not required.
