MDS Coordinator

Job Summary:

A well-established and highly regarded post-acute care organization near Rome, NY is seeking an experienced MDS Coordinator (RN) to join its clinical leadership team. This is a confidential search for a key role responsible for ensuring the accuracy, compliance, and integrity of resident assessments and care planning.

Key Responsibilities:

• Complete accurate MDS assessments and comprehensive care plans

• Initiate and oversee care planning processes and related activities

• Develop and distribute monthly care plan schedules in a timely manner

• Maintain and update all clinical assessments in accordance with regulations

• Conduct ongoing audits of clinical documentation for accuracy and compliance

• Partner with nursing leadership to address and resolve documentation issues

• Ensure strict confidentiality of all resident and organizational information

Requirements:

• MUST HAVE PRIOR MDS 3.0 EXPERIENCE

• Valid New York RN License

• Long Term Care Experience Required!

• Must be highly organized, professional & motivated

• Should have solid computer skills

• Excellent communication skills

• Should be friendly and a team worker
