Grace Unlimited Home Health Care is seeking experiencedPRN RN Case Managers to provide high-quality home healthcare to clients primarily inDallas County, with coverage in surrounding Dallas–Fort Worth areas as needed. Role Responsibilities:

• Provide professional, compassionate nursing care in clients’ homes

• Perform assessments and manage individualized care plans

• Accurately document care usingOASIS/Kinnser

• Manage diverse patient needs in a home health setting

• Respond to PRN and urgent visit needs as assigned

Qualifications:

• Graduate of an accredited nursing program

• Active Texas RN License

• Valid Driver’s License and reliable transportation

• Minimum 1 year of home health experience

• Experience withOASIS/Kinnser preferred

• Ability to travel locally withinDallas County

• Flexible availability for PRN visits

Work Location:

• Client homes throughoutDallas County

• Field-based / on-the-road role
