Position Summary:
HomeCare Maryland is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in their homes throughout Carroll County. In this vital role, you’ll work under the supervision of a Registered Nurse (RN), following an established plan of care to ensure quality, patient-centered home health services.
Your care will support patients who are elderly, disabled, or recovering from illnesshelping them remain safely and comfortably at home.
Key Responsibilities:
Deliver direct nursing care in the home setting in accordance with the RN-developed Plan of Care.
Monitor patient condition and report changes promptly to the supervising RN.
Administer medications and treatments as prescribed.
Perform clinical procedures such as wound care, catheter care, and vital sign monitoring.
Document all patient care activities and observations accurately and timely.
Educate patients and families on ongoing health needs and care techniques.
Collaborate with the interdisciplinary team to ensure continuity of care and positive outcomes.
Provide guidance and support to Certified Nursing Assistants (CNAs) and Home Health Aides as appropriate.
Qualifications:
Current LPN license in the state of Maryland
Graduate of an accredited Practical Nursing program
Current BLS certification (American Heart Association – for healthcare providers)
Valid driver’s license and reliable transportation
Prior home health or med-surg experience preferred
Strong clinical, documentation, and communication skills
Ability to work independently and manage a home-based caseload
