Position Summary:

HomeCare Maryland is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in their homes throughout Carroll County. In this vital role, you’ll work under the supervision of a Registered Nurse (RN), following an established plan of care to ensure quality, patient-centered home health services.

Your care will support patients who are elderly, disabled, or recovering from illnesshelping them remain safely and comfortably at home.

Key Responsibilities:

• Deliver direct nursing care in the home setting in accordance with the RN-developed Plan of Care.

• Monitor patient condition and report changes promptly to the supervising RN.

• Administer medications and treatments as prescribed.

• Perform clinical procedures such as wound care, catheter care, and vital sign monitoring.

• Document all patient care activities and observations accurately and timely.

• Educate patients and families on ongoing health needs and care techniques.

• Collaborate with the interdisciplinary team to ensure continuity of care and positive outcomes.

• Provide guidance and support to Certified Nursing Assistants (CNAs) and Home Health Aides as appropriate.

Qualifications:

• Current LPN license in the state of Maryland

• Graduate of an accredited Practical Nursing program

• Current BLS certification (American Heart Association – for healthcare providers)

• Valid driver’s license and reliable transportation

• Prior home health or med-surg experience preferred

• Strong clinical, documentation, and communication skills

• Ability to work independently and manage a home-based caseload
