The Case Management team is collaborative, compassionate and connected. We prioritize patient-centered care and foster strong interdisciplinary relationships as we work across all departments. Our team is united by a shared commitment to advocacy, empathy, and effective collaboration throughout the hospital.

How you will contribute

A Registered Nurse-Case Manager who excels in this role:

Engages patients and their families in developing a safe discharge plan.

Links the progression toward discharge with the clinical improvement of the patient.

Coordinates clinical care to include discharge planning, medical necessity, length of stay management.

Collaborates with providers, nursing, ancillary staff, patients and families to achieve optimal clinical outcomes.

Executes clear, complete and concise documentation of all interventions and assessments.

Engages in practice that comply with regulatory standards, patient safety and performance improvement initiatives.

What we are looking for

Applicants should have current state RN license and possess an associate's degree. Bachelor's or master's degree is preferred.
