Position Summary:
The role of the ER Case Manager is to perform activities of both Utilization Management and Case Management in the setting of the Emergency Room to facilitate an appropriate status for admission to the Healthcare System and to provide services to those patients who are expected to discharge home or to an alternate level of care. This includes, but is not limited to short-term counseling/crisis management and discharge planning to ER patients and their families who are impacted by an illness or diagnosis, change in residence or lifestyle. This position requires early development of a discharge plan and effective communication to the multi-disciplinary team; therefore, promoting continuum of care and alleviating/reducing anxiety for patients and their families. This role works within the knowledge and regulations of CMS, Commercial Payers, and the UR Plan.
Position Requirements:
Formal Education / Training:
Requires licensure by Tennessee Health Related Boards as a Registered Nurse or Social Work.
Workplace Experience:
5 years Medical Surgical/Cardiac Hospital experience.
Equipment and Skills Training:
Demonstrated organization, time management, problem solving and critical thinking skills.
Able to perform independently and in team situations under the supervision of a Director/Manager.
Demonstrated effective verbal and written communication skills.
Efficient review and interpretation of the medical record.
Familiar with Meditech, basic computer skills required.
Positive attitude with a willingness to learn new processes and adjusts well to change.
Physical Environment:
Office and Clinical, possible exposure to hazardous materials, infectious fluids and airborne particles.
Physical Effort:
Must be able to walk short distances in hospital environment and patient rooms.
Key Results:
Provides psychosocial/crisis/ETOH/drug problem assessment, short term counseling and discharge planning to ER/ICU patients who require inpatient/outpatient treatment within 24 hours or next business day. Identifies resources by specialty and insurance contracts and appropriately documents in Meditech regarding assessment of patient's level of functioning, psychosocial and discharge needs.
Consistently sets priorities and exhibits efficient time management skills with assigned workload within the confinements of scheduled shift.
Timely reviews hospital admissions while in ER using appropriate guidelines for admission status assignments in consideration of the 2MN rule and the intentions of the admitting physician.
Initiates discharge needs assessment on those patients requiring admission 90% of the time.
Timely makes psych placement referrals through Xferral and mobile crisis working with physicians regarding type of placement needed and patient families regarding preferences when appropriate.
Properly identifies legal POA or NOK prior to discussions regarding patient care to maintain continuum of care needs for the patient.
Successfully completes Non Violent crisis intervention, and..
Notifies Hospice organization when known Hospice patients come to ER.
Reports APS/DCS referrals as appropriate.
Short Term counseling with patients and families who are at end of life, lifestyle changes, who are wanting placement or admission when no medically necessary.
Delivers ABN notices or financial responsibility forms when necessary.
Performs within CAHPS standards: ER STANDARD TBD.
